Student Technology Agreement Form
In order to receive a Chromebook for the academic school year, students and parents or guardians MUST complete a Technology Agreement form. Failure to complete and submit a Technology Agreement Form will result in the students not being eligible to receive a Chromebook for at-home use. At DESA, ALL teachers require students to have access to a Chromebook in order to complete assignments and other activities through online learning applications. Completed Technology Agreement Forms should be submitted to the library media specialist, Ms. Vega.
